If you’d like to become a user of the Core Facilities, please complete the below application and send it to the appropriate Facility Manager for the facility which you’d like to use.

To submit samples for analysis please complete the below Chain of Custody form and send it to the appropriate Facility Manager for the facility which you’d like to use. Note that certain requested services when performed by Medgar Evers employees as opposed to you as a facility user will incur Technical Support fees.
